Terms for the Intake Dental Patient Referral Program: earn 5% of a referred dental office's subscription for 12 months, with wallet, cash-out, tax, and disclosure rules.
The Intake Dental Patient Referral Program (the "Program") is offered by Dental Education, Inc. d/b/a Intake Dental ("Intake Dental," "we," "us") to individuals who hold a Patient Passport account and accept the Program agreement in the app. The Program lets you share a personal referral link with dental offices; when an office signs up through your link and becomes a new paying Intake Dental customer, you earn rewards as described below.
You earn 5% of each qualifying subscription payment the referred practice makes to Intake Dental during the 12 months following the practice's first paid invoice. Rewards stop accruing after that 12-month window, even if the practice remains a customer.
Example: if the office you refer subscribes to a plan billed at $149.99 per month, you earn $7.50 for each of its first 12 monthly payments — about $90.00 over the year. A practice on a $499.99 per month plan earns you $25.00 per month, or about $300.00 over the year.
Each practice can be credited to only one referrer: the first valid referral recorded for that practice. A referred practice must be a new Intake Dental customer — offices that are current customers, were previously customers, or that already have a pending signup are not eligible. You may not refer a practice you own, operate, or work for, and self-referrals are void. Intake Dental resolves attribution questions in good faith and its determination is final.
Referral rewards are taxable income. Before your cumulative cash-outs can exceed $600 in a calendar year, US tax rules require us to collect a completed IRS Form W-9 from you; cash-outs that would cross that threshold are held until a W-9 is on file. Where required, Intake Dental will issue you a Form 1099 for the year's payouts. You are solely responsible for any taxes owed on rewards.
Disclosure statement: Participants in this Program have a material connection to Intake Dental — they receive compensation (5% of the referred practice's subscription payments for 12 months) when a dental office they refer becomes a paying Intake Dental customer.
If you promote your referral link — in person, by email, on social media, or anywhere else — you must clearly disclose that you may earn a reward if the office signs up (for example: "I may earn a reward if your office signs up through my link"). This disclosure is required by the US Federal Trade Commission's endorsement guidelines. Do not make claims about Intake Dental that are untrue or that we have not published ourselves.
Rewards are never connected to your dental care or treatment decisions. Rewards are paid by Intake Dental — never by your dental office — and participating (or not participating) has no effect on your care, treatment recommendations, pricing, appointments, or records at any dental office. The Program rewards recommending our software to a practice; it does not reward, and must never influence, any healthcare decision. Your referral link contains no health information, and referring an office never shares any of your health data with that office or anyone else.
Intake Dental may modify, suspend, or end the Program prospectively at any time. Rewards already earned under these terms will be honored. The version of these terms published on this page controls; material changes will be dated above.
